This agreement is between Sacred Openings, LLC (the “Doula”) and the Undersigned below (the "Client") for the purpose of providing birth doula care.
DESCRIPTION OF BIRTH DOULA
Birth Doulas educate and explain all options and assist in creating a birth plan to best support family ideals and decisions. We offer childbirth education, emotional support prenatally and during labor, and physical support during labor. The birth doula’s main role is to support the birthing person (and their partner, if applicable) so that they feel safe, self-assured, and empowered going into labor and the birth space.
DESCRIPTION OF POSTPARTUM DOULA
A postpartum doula is there to help a new family in those first days and weeks after bringing home a new baby. Services include, but are not limited to, help with self-care recovery, postpartum comfort measures, infant care, parenting information and assistance with learning to feed and take care of your baby, as well as other practical services.
Postpartum doula services for a family last anywhere from a few days, up to a few weeks after bringing home a new baby.

Doulas are not doctors or midwives, and do not practice medicine, nor diagnose and treat pregnancy or postpartum related symptoms. Information presented in our meetings and conversations is not intended as a substitute for the medical advice of a properly licensed health care professional. In addition, doulas do not make specific health claims or promise medical results. We provide education and guidance, and childbirth, postpartum, and breastfeeding support.
SERVICE DESCRIPTION
Once you have requested me to act as your doula, I will provide the following services:

I will provide two (2) prenatal meetings. At these meetings, we will discuss and explore your priorities and concerns. We will also explore childbirth education & creating a birth plan, postpartum education & creating a postpartum sanctuary plan, and lactation education & creating a baby feeding plan. Partners must attend at least 1 out of 2 prenatal meetings.

You will receive a digital copy of the Birth, Postpartum, and Lactation educational packets (physical copies provided upon request and for an additional charge).

The Doula will be on 24/7 call for your labor from 37-42 weeks of pregnancy.

Once in labor,
-
Call the Doula immediately, once you believe labor has begun.

If the Doula does not answer, leave a voicemail, and send a text message immediately. Call again every 5-10 minutes for 30 minutes, if the Doula does not respond within that time.

If you have not heard back from the Doula within 30min, wait 1 hour, and then repeat this call cycle again.

If you have not heard back from the Doula by the time that you call to alert her that you are moving to the birth place, call the backup doula.

If both the Doula and backup doula do not respond and subsequently miss the birth,
you will receive a partial refund (see details in "Cancellations & Refunds" section
below)

The Doula will check in every 1-2 hours as labor progresses and will report to the
Client’s birth place once active labor has begun

The Doula will remain with the birthing person until 2 hours after the birth

In the unlikely event the Doula cannot be present at the Client’s birth, a backup doula will be called to support.

I will provide four (4) postpartum visits after the birth.

Once Baby is born, I will provide the following services:

Guidance and assistance with newborn care

Baby feeding assistance/lactation support (assistance with latch, encouragement,
referral to IBCLC, etc.)

Emotional support and nurturing for the new parent(s)

Help with postpartum care (ie: preparing sitz baths, foot soaks, etc)

Placenta Ceremony

Closing of the Bones Ceremony for the birthing parent

Meal preparation

CLIENT RESPONSIBILITIES

Honest communication about any relevant medical or emotional health conditions.

The Client must call the doula once in labor — a text will not suffice.
SCHEDULING
Services are to be provided for up to 3 hours for each prenatal visit, for a total of 2 visits and 3 hours for each postpartum visit, for a total of 4 visits.
Postpartum services are to begin within 14 days of the birth of your baby.
FEES & BILLING
The total cost of services is $3,400.
A non-refundable deposit of 50% of the total payment -- $1,700 -- is due upon the signing of this contract. This deposit will be applied to the total cost of care. The remainder of services must be paid for, in full, by four weeks before the estimated due date.
CANCELLATIONS & REFUNDS
In the event the Client no longer desires Doula services, no refund will be given for termination of this contract.
The Doula does not guarantee birth outcomes. The Doula will support the Client to the best of her ability in any and all birth locations and circumstances, as long as they are safe and her presence is permissible per birthplace policy.
There will be no refund given in the event of an unplanned cesarean section, transfer to a hospital, unplanned use of pain management medications, and/or any and all circumstances that may deviate from the Client's birth plan.
There will be no refund given if the Doula's presence is deemed not permissible per birthplace policy.
If the Client does not call the Doula and she misses the birth, no refund will be given and the full payment will be due to the Doula.
If the Doula does not answer the phone when the Client’s call and subsequently misses the birth, and the backup doula also does not answer the phone when the Client's call and subsequently misses the birth, 30% of the total cost of services will be returned to the Client.
I have read and understand all of the stipulations presented above and agree to abide by them.
